Introduction

XyliMelts (also marketed as AccuDry XyliMelts) are oral adhering discs designed for sustained release of xylitol and other ingredients to moisturize the mouth and reduce oral dryness. Unlike systemic pharmaceuticals, XyliMelts are classified as medical devices or over-the-counter oral care products rather than drugs. They represent a non-pharmacological approach to managing xerostomia (dry mouth) symptoms.

Mechanism of Action

XyliMelts utilize a dual-action mechanism. The disc adheres to oral surfaces (typically between cheek and gum) and slowly dissolves over time. Xylitol, the primary active ingredient, stimulates salivary flow through both osmotic effects and local reflex stimulation. Additionally, xylitol possesses non-cariogenic properties (does not promote dental caries) and demonstrates antimicrobial activity against Streptococcus mutans, a primary contributor to tooth decay. The lubricating agents (including vegetable oils) provide coating and moisturizing effects to oral tissues.

Indications

  • Symptomatic relief of dry mouth (xerostomia) associated with various conditions including:

- Medication-induced xerostomia (anticholinergics, antidepressants, etc.) - Sjögren's syndrome - Radiation therapy-induced salivary dysfunction - Age-related salivary reduction

  • adjunctive therapy in maintaining oral hygiene
  • reduction of oral discomfort during sleep

Dosage and Administration

Administration: Place one disc on each side of the mouth, adhering to the molar or buccal mucosa. The discs may be used during the day or overnight. Dosing:
  • Adults: 1-2 discs as needed for dryness relief, typically every 3-8 hours
  • May be used before bedtime to alleviate nocturnal dry mouth symptoms
  • Duration of effect: Typically 1-2 discs provide relief for several hours
Special Populations:
  • Pediatric use: Safety and effectiveness not established in children
  • Geriatric use: No dosage adjustment necessary
  • Renal/hepatic impairment: No specific recommendations required (minimal systemic absorption)

Pharmacokinetics

Absorption: Xylitol undergoes minimal systemic absorption from the oral mucosa. The majority is swallowed with saliva and undergoes first-pass metabolism. Distribution: Primarily local action within the oral cavity with negligible systemic distribution. Metabolism: Xylitol is metabolized primarily in the liver via the glucuronate-xylulose pathway. Elimination: Absorbed xylitol is eliminated renally with a half-life of approximately 30 minutes. Unabsorbed xylitol is excreted fecally.

Contraindications

  • Known hypersensitivity to xylitol or any component of the formulation
  • Patients with hereditary fructose intolerance (due to xylitol metabolism pathways)

Warnings and Precautions

  • May cause mild gastrointestinal discomfort if excessive amounts are swallowed
  • Use with caution in patients with xylitol allergy
  • Not for use in children under 5 years due to choking hazard
  • Dental examination recommended if dry mouth symptoms persist beyond 2 weeks
  • Diabetic patients should be aware that although xylitol has minimal effects on blood glucose, monitoring is advised during initial use

Drug Interactions

No clinically significant drug interactions have been reported due to minimal systemic absorption. However:

  • May potentially enhance the effects of other oral moisturizing agents
  • No known interactions with systemic medications

Adverse Effects

Common effects (≥1/100):
  • Mild gastrointestinal discomfort (if excessive product is swallowed)
  • Temporary alteration of taste perception
  • Mild mucosal irritation (rare)
Serious effects:
  • Allergic reactions (rare; including urticaria, itching, swelling)
  • Choking hazard if not used properly

Monitoring Parameters

  • Subjective improvement in dry mouth symptoms

  • Dental health assessment (caries, gingival health)
  • In diabetic patients: blood glucose monitoring during initial use
  • Assessment of continued need beyond 2 weeks of regular use

Patient Education

  • Place discs on upper molars or cheek side; avoid placing on tongue
  • Allow disc to adhere naturally; do not chew or swallow whole
  • May be used during sleep for nocturnal dry mouth relief
  • Continue regular dental hygiene practices despite product use
  • Discontinue use if irritation, swelling, or allergic reactions occur
  • Store in cool, dry place away from moisture
  • Not intended for weight control or systemic effects
